NEW WARNING
Celgene Corporation has issued a "Dear Health Care Professional" letter to address updates to the prescribing information for the low-molecular-weight heparin, tinzaparin (Innohep). A controlled study suggests that tinzaparin may increase the risk of death when used to treat elderly patients with renal insufficiency. In July, Celgene revised prescribing information for patients age 90 and older; however, now they believe the risk applies for all elderly patients with renal insufficiency. Celgene suggests prescribing alternatives to tinzaparin when treating elderly patients for deep vein thrombosis (DVT) with or without pulmonary embolism. Tinzaparin is indicated with warfarin (Coumadin) for treatment of acute symptomatic DVT with or without pulmonary embolism.
The new prescribing information also adds epidural hematoma associated with neuroaxial anesthesia or spinal puncture with tinzaparin administration to the "Adverse Reactions" section. This adverse reaction was identified in post-marketing surveillance.
